DNA Collection Information Form

Mail the DNA hair sample (pulled, not cut; 25 to 50 hairs with visible bulbs, taped flat) to the RHBAA with the printed form. Pedigree DNA collection forms must also have the sire and dam fields completed or the form will be returned to the owner. This online form records the information; the hair sample itself must still be mailed.
