RHBAA Horse Show Affiliation Application

Upon receipt of payment ($50.00) and affiliation application, permission will be granted to the judge(s) to serve in the official capacity at your show. Affiliation/sanction forms are to be mailed within thirty (30) calendar days prior to the show. Please include a show bill.

Affiliation fee $50.00 (as printed). Permission will not be granted to any RHBAA judge to officiate until this form is signed and returned.
